Let’s understand the unique anatomy of the shoulder before we dive into why your shoulder hurts. Unlike the hip joint, which is a deep “ball-and-socket” providing high stability, the shoulder (the glenohumeral joint) is often compared to a golf ball sitting on a tee.
The shallow socket allows for an incredible 360-degree range of motion, which is the greatest in the human body. However, this mobility comes at a price, instability. The joint relies almost entirely on a complex system of soft tissues, the rotator cuff, labrum and ligaments to stay in place. When one piece of this puzzle is compromised by injury or wear and tear, the entire system suffers.

This is one of the most hectic conditions a patient can face. Here the shoulder capsule thickens and tightens which physically freezes the joint in place.
- The Three Phases: The Freezing phase (painful), the Frozen phase (stiff but less painful), and the Thawing phase (regaining motion).

Shoulder Ipingement Syndrome
When you lift your arm, the space between the top of your humerus and the acromion (shoulder blade) narrows. In an impinged shoulder, this space is too small, causing the bone to “pinch” the rotator cuff tendons or the bursa sac.
- The Result: A sharp, stabbing pain whenever you lift your arm past shoulder height.
Labral Tears (SLAP Tears)
The labrum is a ring of specialized cartilage that deepens the shoulder socket. Athletes who perform repetitive throwing (baseball players) or those who experience a sudden “pull” on the arm often suffer from SLAP (Superior Labrum Anterior to Posterior) tears.
Shoulder Bursitis
Bursae are fluid-filled sacs that act as cushions between bones and soft tissues. When these become inflamed, the shoulder feels “full” and extremely tender to the touch.
